About phosphodiesterase inhibitors
Both phosphodiesterase inhibitors (PDE5 and PDE6) and (PDE2 and PDE3) have been shown to enhance the efficacy of treatment with selective inhibition of phosphodiesterases (PDEs) and PDE-5. In vitro studies have shown that these inhibitors may decrease the levels of cyclic guanosine monophosphate (cGMP), the PDE of the penile cavernous body. These inhibitors also inhibit cGMP degradation, causing the smooth muscle cells in the corpus cavernosum to relax and allowing the smooth muscle cells to return to normal. In ex vivo studies, these inhibitors were found to be highly selective and not to affect cGMP or its degradation. The PDE inhibitors are not approved for use in the treatment of erectile dysfunction (ED).
As part of a large-scale, randomized, controlled clinical trial, the Journal of Sexual Medicine reported that the use of PDE inhibitors to treat ED caused a decrease in the mean number of erections in the study group over a period of 12 months. The PDE inhibitors were found to be effective in reducing the duration of erections. PDE inhibitors are currently the second-best phosphodiesterase inhibitor in men, after selective phosphodiesterase inhibitors, and have been shown to be as effective as PDE-5 inhibitors.
In a, a randomized, double-blind, controlled study of 18 patients, all taking tadalafil, compared the efficacy of PDE inhibitors with that of selective phosphodiesterase inhibitors (PDE5 and PDE6). The mean duration of sexual activity was 2.9 and 3.8 months in the treatment group and in the control group, respectively.
The results of the study, published in, demonstrated that tadalafil was effective for all patients. The mean time of ejaculation was 3.6 and 4.5 months in the treatment group and in the control group, respectively.
Although there were no significant differences in the effectiveness between the two groups, tadalafil did produce an overall decrease in the number of erections. In the study, a mean reduction of 4.7 and 4.2% in the first and second episodes of erections, respectively, was seen in the tadalafil group, compared to a reduction of 4.1 and 4.4% in the control group. However, tadalafil did not produce an overall decrease in the duration of erections.
The authors of the study concluded that tadalafil could be considered as a more effective and safe PDE5 inhibitor in men with erectile dysfunction, with a lower risk of developing the condition, and that this was achieved by a better understanding of the chemical structure of the drug and the underlying biological pathways involved. A recent review of the literature concluded that both PDE inhibitors and tadalafil were effective and safe in treating ED. This was based on a review of the literature and on a systematic review of the literature.
The authors of the study conclude that “in vitro” studies in animal models have shown that tadalafil has a higher potency than sildenafil. Animal studies indicate that sildenafil is more effective than tadalafil in treating ED and in reducing the frequency of erections.”
